Will Sexual Intercourse Help in Expulsion Distal Ureteral Stones

Dear All,

We had extensive Discussions on MET for Ureteric Calculi in Uroeducation and as well in many Urology meets at all levels over the past years. There have been many articles addressing the benefits of MET for Ureteral calculi especially distal calculi <10mm. But there are studies indicating that they are not that effective as presumed to be. Recently there are articles indicating combination of drugs that can be beneficial in Stone Expulsion than single drug.

I am sure many in our group have diverging views on this and hopefully will come out with their opinion in this discussion.

Does Sexual Intercourse help in Expulsion of Ureteral Stones especially those in Distal Ureters? Currently Sexual Intercourse is regarded as a Novel Therapeutic Option for MET of Distal Ureteral stones.

This issue was addressed in 2015 by Omer Gokhan Doluoglu et al. They first demonstrated that sexual intercourse 3 or 4 times per week increased the spontaneous passage of ureteral stones, reduced the analgesic requirement, and shortened the expulsion time (PDF of the article Provided).

Most of these articles are addressing the effectiveness of Sexual Intercourse in Males for Expelling Ureteral Stones. Hassan Tugut (2020) has published a prospective RCT on the Efficacy of Sexual Intercourse in Women for Expulsion of Distal ureteral calculi.

What do all these indicate? It is well known that Waitful watching, by itself can expel Distal Ureteral stones in both males and Females. This has been one of the objections regarding MET as well. Though it has been mentioned that MET offers an early expulsion but in many the calculi will pass out, knowingly or unknowingly in many without any medications.

Hence this concept that is now being addressed as a novel therapeutic option is really worth trying in married men and women or the whole concept is a myth and not really to be considered.

I would appreciate our members to place their views on this so called Novel Therapeutic Option.

  Sexual intercourse 3 to 4 times a week is as effective as tamsulosin in treating lower ureteric stones. This is a good observation. 

  PD 5 inhibitors are known to be effective in lower ureteric stone expulsion. A comparison with sexual intercourse versus PD 5inhibitor should provide more compelling evidence regarding the effectiveness of sexual intercourse in lower ureteric stones.
