Unsual Foreign body in urethra

Dear all,

Of all the urological emergencies, most dramatic and technically challenging is removal of foreign bodies inserted into genitourinary tract.various foreign bodies are inserted for various reasons.urologist should evaluate the case thouroughly and be able to understand the circumstances accordingly.common reasons being sexual gratification,drug paddling,sexual abuse, self punishment etc.
Here is a case of copper wire inserted via urethra in a male who has history of psychiatric illness and on treatment.prompt identification and cystoscopic removal was done under anesthesia without any complications.

    Dear All,

    Nanda has posted a Foreign Body in the Urethra. 

    We had an Extensive Discussion when we were with Uroeducation in Yahoo Group on this topic way back in 2010 where I had provided considerable literature on the topic.
    Foreign bodies have been inserted as the result of curiosity, autoerotic stimulation, or medical procedures. The material of the foreign bodies has varied from wire, screw, or ball point pen to animals such as a snake.
    Causes of foreign bodies in lower urinary track include psychological, iatrogenic during urological procedure, traumatic aspect and migration from other organs. In psychological aspect, various circumstances including exotic impulse, mental illness, borderline personality disorder, sexual curiosity, sexual practice while intoxication and so on make self-insertion of foreign bodies in lower urinary tract. Among these, the most common motive for foreign body insertion in the lower urinary tract is sexual or erotic in nature, such as masturbation or other forms of sexual variation or gratification. Various materials such as paraffin, a urethral device, metallic cables, plastic tubes, fish, and snake have been reported as foreign bodies in the lower urinary tract.

    My good friend Prof. Mohanchandra Suvarna had provided me with a case way back in 2010, which I had posted during my earlier post also. 
    56 yrs old lady presented to Mohan with Dysuria and Intense frequency of urination 8 months duration and difficulty in voiding. She used to get fever low grade on and off relieved with antimicrobials. There was no relevant past history.  The PDF pictures are being sent of US, and Cystoscopy Images. These showed a large calculus in the bladder over a foreign body. The foreign Body in this case was a small wad of Rs 20 notes. (PDF provided). In none of the reported cases, Currency notes have been reported as a cause.

    This is worth watching as such a Fish in its complete form has never been reported. 
    Ascaris lumbricoides as a cause for Urinary Retention has been presented by D Singh er al in 2010.

    PubMed has cited 1236 articles on the subject till date. Most of them are case reports with few having a series.
    I am sure there will be many in our group might have seen unusual Foreign Bodies in the Urinary Bladder and placing it in Uroacademy could be help for all. I remember Anil had posted a Video on this at the previous discussion. I do not know whether he has with him even now.

    In my reply to Nanda’s post, I had provided few references of importance as regards Foreign Bodies in Bladder. As I had problems with posting the reply in Uroacademy, I had to request Anil to do so. I have given an attachment of MohanChandra Suvarna’s case which is available for all to see.
