Recurrent febrile UTI with Bilateral VUR

Dear All,

20 months old male child presented with history of recurrent febrile UTI since last 6 months.
Child is on AB prophylaxis since 4 months before that received AB from 2 more pediatricians who failed to recognise PN responsible for pyrexia.
At present;
Urine: 8 to 10 pus cells, no protein, no rbcs
Last culture was positive for E. Coli
S. Creatinine: 0.4 mg%
HB: 9.7
I am attaching recent USG, VCUG and DMSA scan done in last 7 days.
Previous USG not available to post.
Please discuss; diagnosis, further investigations and treatment...
Thanks
