Is it PU Valves?

Five day male neonate. Prenatally detected right HUN. No voiding trouble. Postnatal USG: Right HN HU, parenchyma thin. Bladder is smooth walled. This is the MCU. Will you consider diagnosis of posterior urethral valves on this MCU.

    Dear Prashant,


    No, this is not a case of PU valve;
    1. Prenatal only Rt. HUN, nothing suggestive of keyhole sign in USG. Liquor is adequate I presume.
    2. Urinary stream is good according to you and mother
    3. bladder is not palpable
    4. In MCU; bladder wall smooth, no posterior urethral dilatation, entire urethra appears normal, no bladder neck hypertrophy some pathology is seen at bladder neck region, can be because of ectopic insertion of right dilated ureter.
    5. In absence of reflux in MCU, my probable diagnosis is right HUN with possibilities of dilated, ectopically inserted ureter...
    I would like to recommend repeat USG after couple of weeks

    Thanks a lot Anil for the comments. The patient was referred to us for second opinion as other consultant had a suspicion of PU valves. We removed his catheter and is doing well. Planning to do his EC scan with ROI across right UVJ

    i would agree with Dr Anil sir regarding this and would not consider puv as wonderfully pointed out in his discussion..
